Community Habilitation is a Medicaid-funded program operated under OPWDD to provide one-to-one training to people with (IDD) intellectual/developmental disabilities to develop or enhance the skills needed to live more independently in their homes or in the community.

  • Services are provided in a person’s own home or family’s home by our dedicated staff working one-on-one to help each person reach their personal life goals.
  • From learning daily living skills to navigating social settings or participating in community activities such as going to the gym, the movies or religious services. Com Hab promotes choice, growth, and meaningful connection.
  • Learn to drive, shop, cook, pay bills and take care of your home!

ComHab-R

Com Hab–R offers personalized one-to-one or small group support for individuals living in certified residential settings like IRAs or Family Care.

Services are designed for those 65+ seeking retirement from day programs or those with complex needs. Support focuses on daily living skills, social engagement, personal goals, and community inclusion, all delivered by trained, compassionate staff centered on each person’s choices and strengths.
